Bracknell Forest Council migrated its Intermediate Care Service from CM2000 to DOM Portal in just 10 weeks — going live 14 days ahead of contract deadline.

The Challenge

Bracknell Forest Council's Intermediate Care Service needed to replace Access CM2000 before its contract expired in September 2026. The Council required a modern, cloud-hosted Electronic Tracking and Monitoring System (ETMS) supporting community-based care workers with real-time care records, medication management and goal tracking.

  • Time pressure: a fixed September deadline with no room for delay.

  • Data migration: historical care records and service-user data had to transfer without disrupting continuity of care.

  • Staff readiness: approximately 100 care workers, coordinators and managers needed to be trained before launch.

  • Security: staff needed Single Sign-On using existing BFC Microsoft credentials.

Wigan Council embarked on a significant digital transformation journey by selecting Dom Po

The Solution: DOM Portal

A structured implementation plan was agreed at project kick-off, with five milestones from contract signature to go-live. The joint BFC–DOM Portal team met regularly, and DOM Portal provided dedicated on-site training at the Council's Bracknell offices.

Timeline

  • 5 June 2026: contract signed.

  • 15 July 2026: CM2000 data migration completed.

  • 28 July 2026: three on-site staff training sessions completed.

  • 14 August 2026: service rehearsals completed.

  • 21 August 2026: BFC ICS DOM Portal go-live.

Total elapsed time from contract signature to go-live: 10 weeks. The contractual deadline was 4 September 2026, so delivery was achieved 14 days early.

Project Approach

DOM Portal is a cloud-hosted digital care management platform purpose-built for UK social care. For BFC's Intermediate Care Service, DOM Portal delivered:

  • Digital care records: real-time records on Android, iOS and desktop.

  • eMAR: medication administration and digital sign-off.

  • ADLs and goals: assessments and goal tracking available in the field.

  • Reablement workflows: case management and outcome recording.

  • Referral management: intake and allocation within the platform.

  • Single Sign-On: existing BFC Microsoft credentials on mobile and desktop.

  • Migration and security: validated CM2000 history, GDPR-compliant hosting and full audit trails.

Outcomes: First Three Weeks Post Go-live

The first three weeks demonstrated a safe and effective transition with zero disruption to care delivery.

  • 105 new referrals processed.

  • 78 Reablement cases completed.

  • 0 medication errors reported.

  • 0 care-delivery issues reported.

  • 4.52 / 5 staff satisfaction rating from 25 responses.

  • 100% of surveyed staff trained before go-live.

Training and rehearsals

All 25 surveyed staff confirmed they had received training. Six requested optional refreshers, primarily on eMAR and goal sign-off. Full rehearsals used a test environment pre-populated with migrated data, allowing staff to practise visits, care notes and eMAR workflows before live records were accessed.

Key Takeaways

  • 10-week delivery: data migrated, 100 staff trained and go-live achieved 14 days early.

  • Zero disruption to care: no medication errors or care-delivery issues in the first three weeks.

  • Strong adoption: 4.52/5 satisfaction, with staff describing the system as streamlined and user-friendly.

  • Mobile-first care: field staff access care plans, ADLs, goals and notes in real time.

  • Seamless SSO: BFC staff use existing council credentials.

  • Long-term partnership: a seven-year contract provides a stable, evolving platform through May 2033.

DOM Portal — Empowering Care. Enhancing Lives.

Client Perspective

“Our evaluation process identified more providers of software for domiciliary care visiting officers than we had been aware of. We evaluated more than six providers and the DOM Portal care system scored highest on the quality part of our procurement process. The price was also very competitive, and we have achieved significant financial saving going forward. We quickly established a rapport with the DOM Portal team who were professional, responsive and flexible on aspects of the BFC system set up.”

“We now have a more modern system with enhanced features and improved integration with other systems in use at BFC. All in all, it’s been a very successful project, and we look forward to doing more with the system and the supplier to further streamline administrative processes.”

— Ken Conolly & Andy Trotter, Programme & Project Managers, Bracknell Forest Council

“The new system we now have in place is modern, intuitive, and fit for purpose. The functionality available has significantly improved our ability to manage and oversee service delivery.”

— Lauren Ross, Assistant Team Manager, Community ICS, Bracknell Forest Council

“When out in the community and having an unexpected visit, we can read them on our phones, so we know what we are doing and how they are. Also being able to read ADLs and goals on our phones is very beneficial.”

— Community Care Worker, Bracknell Forest Council
